1. Caring for patients experiencing acute mental distress or who have an enduring mental illness.
2. Assessing and talking to patients about their problems and discussing the best way to plan and deliver their care.
3. Building relationships with patients to encourage trust, while listening to and interpreting their needs and concerns.
4. Ensuring the correct administration of medication, including injections, and monitoring the results of treatment.

1. Support the registered nurse in implementation of an agreed plan of care with the patient and in accordance with instructions and training received.

2. Assist in the formal supervision of other carers.
3. Assist the trained nurse in the appraisal of other carers.
4. Take on the role of key worker for a group of clients.
5. Assist a trained nurse in the administration of medicines.
6. Respect the individual rights and choices of clients.
7. Respond to the differing cultural values and diversity of clients.

15. Supervise clients at mealtimes ensuring that they have a satisfactory intake of nutrition consistent with their preferences and needs.
16. Undertake generic cleaning duties particularly at night.
17. Dispose of waste safely and in accordance with care home policy.

1. In all his/her duties the Care Manager must work within the policies, procedures and statutory requirement of Excellent care.
2. All Care Managers will be required to undertake additional on-call duties over night from home and/or on site if home address is more than 10 miles away.
3. All Care Managers will be expected to be ‘on call’ as per the Care Managers roster and deal with all eventualities arising. In any serious emergencies s/he should liaise with the Senior Management person on duty.
4. As a member of the Care Management team, you will be expected to deputise for the Head of Care in his/her absence.
5. In the absence of designated Safeguarding Officers and/or on-call member of the Senior Management Team, the rostered Care Manager will take the lead on any Safeguarding matters.
6. To assist with the development of policies, procedures and guidelines regarding care delivery.
7. To attend all relevant organisational meetings.
8. Liaise with all other departments as appropriate.
9. Implement budgetary systems in accordance with operational planning and financial instructions.
10. Implementation of appropriate fire procedures.
11. Ensure appropriate staffing levels are maintained.

1. To ensure requests from commissioners are responded to effectively, working within contractual arrangements.
2. To ensure client assessments, care plans and reviews are completed within the guideline timeframes and to the standard required.
3. Carry out appropriate Risk Assessments and Health and Safety Assessments as required and adapt services accordingly to ensure that the safety of clients and staff are of highest priority.
4. To ensure any complaints, concerns or incidents are responded to appropriately in line with Crossroads policies and procedures.
5. Ensure that reassessments of care are implemented, outcomes are monitored and client records are updated, as per organisational time scales.
6. To be aware of other services locally and make referrals to other agencies as appropriate.
7. Alert appropriate agencies of any concerns about the care of vulnerable adults who may be at risk and follow up / monitor according organisational guidelines.
8. Attend case conferences and statutory reviews relevant to the family as required.
9. To have a working knowledge of Excellent Care’s policies and procedures, national and local policies, guidelines and standards, in relation to Care Services.
10. Ensure implementation of procedures and policies within the Scheme and Care Teams are aware and understand the implications of change.
11. To represent the Excellent Care Services at provider meetings and other required events and liaise with managers to feedback information and embed new initiatives into practice across the service.
12. To communicate effectively with members of staff and other agencies involved in the care and wellbeing of carers and people with care needs.
13. To ensure that all staff and the Operations Manager are kept informed of current activities and specific issues, as required.
