1. In all his/her duties the Care Manager must work within the policies, procedures and statutory requirement of Excellent care.
2. All Care Managers will be required to undertake additional on-call duties over night from home and/or on site if home address is more than 10 miles away.
3. All Care Managers will be expected to be ‘on call’ as per the Care Managers roster and deal with all eventualities arising. In any serious emergencies s/he should liaise with the Senior Management person on duty.
4. As a member of the Care Management team, you will be expected to deputise for the Head of Care in his/her absence.
5. In the absence of designated Safeguarding Officers and/or on-call member of the Senior Management Team, the rostered Care Manager will take the lead on any Safeguarding matters.

5. Ensure that reassessments of care are implemented, outcomes are monitored and client records are updated, as per organisational time scales.
6. To be aware of other services locally and make referrals to other agencies as appropriate.
7. Alert appropriate agencies of any concerns about the care of vulnerable adults who may be at risk and follow up / monitor according organisational guidelines.
8. Attend case conferences and statutory reviews relevant to the family as required.
9. To have a working knowledge of Excellent Care’s policies and procedures, national and local policies, guidelines and standards, in relation to Care Services.
10. Ensure implementation of procedures and policies within the Scheme and Care Teams are aware and understand the implications of change.
11. To represent the Excellent Care Services at provider meetings and other required events and liaise with managers to feedback information and embed new initiatives into practice across the service.
12. To communicate effectively with members of staff and other agencies involved in the care and wellbeing of carers and people with care needs.
13. To ensure that all staff and the Operations Manager are kept informed of current activities and specific issues, as required.
